Our Mission StatementSangre de Cristo Hospice & Palliative Care (SCHPC) provides terminally ill patients with the necessary physical, emotional and spiritual support to ensure the best quality of their remaining lives, allowing them to die with dignity. Additionally, hospice provides bereavement services for members of the community. Your GiftAs a nonprofit organization, we depend on the generosity of our community members. It is through donations, memorial gifts, bequests and planned giving that we are able to provide comprehensive and compassionate end-of-life care to terminally ill patients, regardless of their ability to pay. Our ServicesWe use an interdisciplinary approach to caring for patients and their families facing serious or terminal illness. Each patient has a team of dedicated hospice and palliative care professionals who work closely with him/her and their family members, ensuring comfortable compassionate care. Our Care TeamsOur teams of physicians, nurses, chaplains, certified nursing assistants, counselors and volunteers ensure that pain control, symptom management, medications, medical equipment, caregiver education and other resources are provided to our patients and their families under their personalized care plan. “Promise of Spring” Quilt Raffle
